- Approved grantees MUST follow grant guidelines for use and administration. Contact our office if you are unable to use your grant funds for any reason.

CareLink Caregiver Support
Grant
Reimbursement Grant
- Person is over the age of 60 with any dementia or chronic illness diagnosis requiring a caregiver for support.
- A diagnosis letter is required, even when applying on-line.
- Person living with dementia lives in Pulaski, Lonoke, Saline, Faulkner, Prairie, or Monroe county in Central Arkansas
- Person with dementia lives independently or with family (not in assisted living or a full-time care facility)
- This grant funding is a reimbursed grant. You must complete a log showing you have spent respite funds for us to send the grant check OR have a company invoice us directly for their services.

Dementia Caregiver Respite Grant
$500 Grant
- Person living with dementia can be any age living in Arkansas
- A diagnosis letter is required, even if applying online.
- Person living with dementia lives independently or with family (not in assisted living or a full time care facility)
- You must complete a respite log once funds are spent to qualify for future grants.

Family Assistance Program Grant
$500 Grant
- Person can be of any age with an official diagnosis of Alzheimer’s or some other type of dementia living in Arkansas.
- Person can be over the age of 60 with any chronic illness diagnosis.
- A diagnosis letter is required, even when applying on-line.
- Person lives independently or with family (not in assisted living or a full-time care facility)
- You must submit a respite log once funds are spent in order to qualify for future grants.
